We have many report generating tools around, but not specifically for a store manager, who can jot down the details about the store during his visit.

All he needs to do, is fill up a form which contains 18 questions, and one needs to upload 5 photos for each question. Also, he needs to select a smiley which defines the health of the store. Based on manager's input, managing team will get an idea what improvement needs to be done.
