Please note: The first version of MijnPortavita is only available for patients who are affiliated with the thrombosis service Starlet DC from Alkmaar, thrombosis service Apeldoorn and Zutphen (Gelre hospitals) and Stichting Doctoren Laboratorium en Thrombosedienst (SALT).

The MyPortavita app gives you quick access to your anticoagulant data. See immediately the number of tablets you need to take for today and what your next injection date is.

The MijnPortavita app supports, among other things:

- the number of tablets to be taken for today

- the next injection date

- the complete dosing schedule

- the course of the INR values

- the most important medical data

- the messages from your Thrombosis Service

- the contact details of your Thrombosis Service

- the possibility to check off taken tablets (next version)

The MijnPortavita app can only be used by patients who are affiliated with an organization that uses the Portavita Health Management Platform Anticoagulation.
