This application is an Appointment Planning Tool for the APT Teen Study through Yale University. The goal of the APT Teen Study is to see if an appointment planning tool helps adolescent girls communicate better with their healthcare providers, get their health questions answered, and get the services they need. Participation requires completion of surveys, and the study participants are already aware of these requirements.
