Added Version number and app name to get entries

Not allowing the user to change judge type if in judging mode or NoPush
Mode; the judge must enter the judge number. If in practice mode the judge
must select judge type and cannot enter the judge number

Fixed index out of bounds; -1 on the athlete screen
