NHS Hartlepool & Stockton CCG

This HAST CCG (NHS Hartlepool & Stockton Clinical Commissioning Group) 'My Health' app has been developed to offer patients a range of health and well-being services direct to their smartphones. Core health information such as local services (GPs, Hospitals, Walk-in centres and much more) sit alongside health advice from both NHS Choices and NHS Direct. Patients in the Hartlepool and Stockton area can also use a range of tools such as the NHS Choices Symptom Checker and BMI Calculator to further understand their health issues.

We've also included a range of other useful tools including Universal Jobmatch from JobCentre Plus, Universal Credit and The Money Advice Service.
