Updated to support android 6.0 and the new permission model.

Removed support for android 2.3 and android 3.x
There were only a couple users with those versions and it was a lot of work to maintain support.

Fixed an issue with picking custom ringtones either not showing up at all or not showing the correct name. This required a new app permission. It is used only by the ringtone picker.
