Healthcare providers will find guidelines on hospice eligibility along with an electronic signature feature to expedite their hospice referrals. The app also provides answers to frequently asked questions and showcases the services of Community Hospice. Community Hospice’s Referral and Admission Center will respond to referrals submitted through this app. All patient information received through the referral form is HIPAA compliant.
