1.- Un archivo de excel (2003) con un formato especifico, este contendra un tipo de control, nombre del campo en la base de datos y una etiqueta para los controles.

2.- Tambien se requieren unos archivos que incluyen los archivos de PHP ó JSP, JavaScript y Bootstrap, pongo el link de descarga, descomprimir los archivos y la carpeta renombrarla con lo que tecleaste en el nombre del projecto.

3.- copiar los archivos que genero la app y ponerlos dentro de la carpeta que descomprimiste en el paso anterior.

4.- Subir todos los archivos a tu servidor web.
