SHIFT Prognostic model

The SHIFT prognostic model provides prognostic information in ambulatory patients with chronic systolic heart failure receiving contemporary guideline-recommended therapy.

Ten parameters predictors for risk stratification in patients with chronic heart failure and left ventricular systolic dysfunction, including easily obtainable clinical characteristics have been included in this model.

The calculator assesses, based on the Ford I., Robertson M., Komajda M. et al. Top ten risk factors for morbidity and mortality in patients with chronic systolic heart failure and elevated heart rate: The SHIFT Risk Model. Int J Cardiol. doi: 10.1016/j.ijcard.2015.02.001 , the risk of cardiovascular death or hospital admission for worsening heart failure over the next 2 years, and the risk of hospital admission for worsening heart failure over the next two years.
