This app is a tool to collect information from individuals in the community seeking support for health problems and other social issues. It is to support community volunteers reach vulnerable individuals and their households, and to provide them with all the necessary support possible. Here are some of the App features: - Collect the contact information of vulnerable households - Undertake health screening - Recording information and relaying it to appropriate stakeholders (such as hospitals, local government, etc).
