What is Nursing care plans?

A Nursing care plan contains information about the patient's diagnosis, treatment goals, specific care instructions (including observations needed and actions to be taken), and an evaluation plan.

The plan will be updated with any changes and new information as the patient's hospitalization progresses. In fact, most hospitals require nursing staff to update care plans during and after each shift.

Types of Nursing Care Plans

There are four main types of care plans.

🧩 Formal: This is a written or computerized plan for organizing and coordinating a patient's care information and care plan.
