Patient App: Patient app helps the patients to record health vital readings using self- monitoring devices. Once measured, the captured health vital data seamlessly transfers to Patient app and syncs with Provider app and web portal in real time for further review and action. Patient app also helps the patients to view the trend of the health vital readings that are recorded in the app and track medicines schedules/ compliance prescribed. The patient app also allows patients to communicate (text/voice/video) with their care team members and/or their family members when required.
