CTCA Continuous Care

The CTCA Continuous Care App will support your participation in our health and wellness monitoring programs.

The App will help you stay aware of your activity, sleep, vitals, and other features specific to the wearables and other monitoring devices you are using with your program.

You may also be able to communicate with your care team, set up reminders, privately connect with others, and more.
