1. Your Phone Number: so Nexiwave can identify your voicemails and text you the transcript;

2. (Or) Your email address: if you so choose to receive voicemail transcript via email;

3. Name of your mobile carrier: so Nexiwave can choose appropriate call forwarding instructions for your carrier.
