ChoiceMMed Health

ChoiceMMed Health is a Patient and Caregiver app for ChoiceMMed Cloud Patient Monitoring System. It designed to be used by patients and caregivers enrolled in managed care plans. It provides the following functionality:

* Vitals collection from compatible iChoice medical devices

* Trend and threshold analysis

* Data synchronization with the managed ChoiceMMed Cloud platform.

The app is not available for individuals not enrolled in managed care plans. Please contact your managed care provider for the access code to use the app.
