DTD is a psychological injury that results from protracted exposure to prolonged social and/or interpersonal trauma in the context of dependence, captivity or entrapment (a situation lacking a viable escape route for the victim), which results in the lack or loss of control, helplessness, and deformations of identity and sense of self. Structural brain studies of DTD show reduced gray matter volume in fear regulatory areas including the ventromedial prefrontal cortex, the occipital cortex, the anterior cingulate gyrus, the superior medial frontal gyrus, the insula, the parahippocampal gyrus, the amygdala, the orbifrontal cortex, the superior parietal lobule and hippocampus.

White matter decreases are found in corpus callosum area.
