Siaya County in Kenya implements the community health strategy. The county is in collaboration with Medic Mobile to implement mobile application to support the community health workers in their routine data collection and case management. This application supports 4 use cases; Antenatal care (ANC), postnatal care (PNC), Immunization and integrated Community Case Management (iCCM). ANC focuses on a pregnant woman’s attendance to the ANC appointment. The tool provides alerts to the Community Health Volunteer (CHV) to remind the woman of upcoming visits to the health facility. The main indicators of interest are the number of women who are enrolled into ANC within the first 16 weeks of pregnancy and the number of women who complete the 4th ANC visit. PNC focuses on the period after delivery ensuring that a woman attends all the required visits at the health facility. The main indicator of interest is the number of women attending at least 3 PNC visits at the health facility. Immunization facilitates reminders to the mother through the CHV a child attends all the required immunizations sessions. The main indicator of concern is the number of children fully immunized by the first year. iCCM focuses on cough, fever and diarrhea for children between 2months and 5 years of age in the effort to reduce the mortality rate for children under 5 years of age.
