Four Seasons

This referral app provides a convenient way for healthcare providers in WNC to refer hospice or palliative care patients to Four Seasons Compassion for Life, serving Buncombe, Henderson, Macon, Jackson, Transylvania, Swain, Haywood and surrounding counties. This app also provides a video, photos, and information about additional services and programs offered by Four Seasons. Healthcare providers will find guidelines on hospice or palliative care eligibility.

Referrals submitted through the app will be received by Four Seasons’ Patient Access Department for expedient follow-up. All patient information received through the referral form will be managed with HIPAA compliance.
