MACE Concussion Evaluation

The Military Acute Concussion Evaluation (MACE) is a concussion screening tool for the acute assessment of service members involved in a potentially concussive event.

This application walks the user through the 02/2012 MACE. Application will then generate a PDF that can be digitally signed or left blank. If left blank, the user can sign the printed document with a pen, which is recommended by the developer (Duy Dang). The complied text will be pasted into the body of a generated email.
