CoCare

Care workers, council staff, NHS staff, family members and informal carers can use the app to record visits and communicate with each other.

There are notes for care professionals as well as messaging for the whole care network that keeps loved ones in the loop.

CoCare also supports the recording and sharing of information about wellbeing, personal goals, health conditions, social care needs and the experiences of people receiving care.

CoCare has been developed by Kingston Council, to support an outcomes-based commissioning approach, and is currently only available for use by their partners.
