InCare for Community Resources

InCare for community resources is Innovaccer’s brand new mobile application that enables community resources and social workers to close the referral loop of patients who are socially vulnerable. Community resources can register themselves on the app and receive referrals from multiple health systems that are on Innovaccer’s platform.
