CoPHR

Health First Colorado (“CoPHR”) is a mobile application that helps user to access their personal health records and other useful information. The app can help user to view their personal profile and medical health information, such as Person-centered plan, assessment, hospital encounters, and units of services etc. It also enables user to create calendar events to track medical or personal appointments. The app is integrated with the Health First Colorado Patient Health Record web portal used by Colorado Department of Health Care Policy & Financing (HCPF) and by Colorado Regional Health Information Organization (CORHIO).
