Finding Patient Groups

“Finding Patient Groups” is the smart phone version of “Find a patient group” of Smart Patient Website. We would like to encourage patients and carers to use this application to access to contact information of patient self-help groups anywhere and anytime for sharing experiences and care tips with peer.

The Smart Patient Website (SPW) (http://www.smartpatient.ha.org.hk) was launched in 2009. It is a one stop information website especially designed for patients and carers to access accurate information which enhances their knowledge of disease and community resources, so as to improve their self-care skills and strengthen their ability on disease management.

We strive to provide practical health and disease information to patients and carers. Should you have any suggestions on this application, please contact us via [email protected]. Thank you.
