PocketPK - Vancomycin Dosing Calculator

PocketPK is a pharmacokinetic app for medical professionals to aid in dosing IV vancomycin therapy for their patients. Within the app, loading and maintenance doses are suggested based on 5 important patient parameters: age, gender, weight, height and serum creatinine. Dosing regimens are suggested based on population kinetics of non-ICU hospital inpatients utilizing dosing weight, but there is also an option to dose based on known peak and trough levels, enabling the user to tailor vancomycin therapy specifically to the patient in front of them.

Recent clinical guidelines have suggested that clinical efficacy of vancomycin is optimized when the pharmacodynamic parameter area under the curve over 24 hours (AUC24) is between 400-600 mg*L/hr. PocketPK takes this into consideration with dosing recommendations and provides a visual representation of the kinetics of the drug, both with and without loading doses. It also provides a predicted peak, predicted trough and AUC24.

A final feature is the ability to model patient parameters in a time to delay dose in the case of a supra-therapeutic trough. Using patient parameters with the supra-therapeutic trough measured and a desired trough, a timeframe is suggested (in hours) to wait before scheduling the administration of the next vancomycin dose.

PocketPK is intended to be used by physicians and pharmacists to standardize how vancomycin is ordered in an attempt to improve patient safety and clinical efficacy.
