Pak Patient Registry

Pak Patient Registry is nation building initiative for establishing country wide patient registry. It requires minimum information and can be entered by the doctor or by an assistant on doctors behalf. It is kept simple for wider uptake by professionals. The most important aspect is ease of accessing the ICD 10 disease coding so that diseases being reported can be standardised. It will help in gathering very important data used for planning and information sharing.

Patient consent is mandatory for reporting.
