Add caller id (if known from contacts) to entries in custom incoming
calls log.

Fix twice-recorded entries in custom incoming calls log.

Make and keep a list of last received call numbers instead of using
call log, as READ_CALL_LOG permission is now denied (but we only get
incomings calls received after the app was started).
