This is only for people participating in a rehab program that offers Cardihab as a mentoring delivery option.

Cardihab assists with the delivery of rehabilitation and chronic disease programs by a health service to people who can not attend traditional in facility programs. These programs are generally focused on changing peoples lifestyle to a more healthy and active one.

It gives clinicians the ability to access and monitor their cohorts disease risk factors information as it is collected. This allows for review before they enter the clinic or are called for a scheduled mentoring session. This includes reporting of symptoms.

Activity can also be tracked if it is relevant to the rehab program.

Educational information in the form of video and scheduled notifications are also available.
