Health Coach

Health Coach will guide you through your health journey by offering you a personalized plan with daily activities that will continuously be adjusted by your own plan and your care team.

 The personalized plan may include valuable information, helpful videos, things to do, special health trackers and fun quizzes. It will also connect with your Apple Watch, Fitbit and other wearables. The app will actively remind you of your appointments, medication times, and many more activities as it will follow your journey.

 You would also be able to communicate with your care team at any time by chat messages or through video. The app also offers a group coaching option that will allow you to share and get support from other people in the program.

 This application is provided for free by your sponsoring organization. Not all features will be enabled by your organization.
