iUGO Care helps those with disabilities and chronic conditions manage their health independently and prevent unnecessary exacerbations and hospitalizations, empowering individuals of any age to live safely in the home of their choice.

iUGO Care is a real-time electronic chronic disease management and communications platform allowing patient to receive secure, high quality care at home. With iUGO Care patients can communicate with care providers and share biometric readings from a wide range of digital biometric devices like glucometers, blood pressure cuffs or fitness devices. Patients, caregivers and the care team can track and visualize patients' health status, and securely message one-another in real time. Assigned care team members receive alerts when patients' status trends towards instability, enabling early intervention, just-in-time coaching, and avoiding unnecessary exacerbations and hospitalizations.

The iUGO Care platform is only available to subscribed patients invited by participating physicians and clinical programs.
