The Spanish Society of Rheumatology (SER) and the Spanish Society of Family Medicine and Community (SEMFYC) have jointly developed a guide on systemic autoimmune diseases to facilitate referral to a rheumatologist by the primary care physician, in order to detect and refer any suspicion of such conditions.

This initiative is coordinated by Dr. Ana Arana Urruticoechea, rheumatologist.; and Dr. Fernando León Vázquez, family doctor. And on the editorial board doctors Iñigo Rúa-Figueroa, Jose Luis Andreu, Alejandro Olivé, Mercedes Freire and Vicente Giner involved.

The main objective of the project is to develop a referral guide that will help the primary care physician in decision-making at the clinical suspicion of an EAS, facilitating their recognition and referral to a rheumatologist in your case.

The secondary objective is to sensitize family physicians of the existence of these less common diseases, improve the quality of referrals, reduce waiting times and coordinate the two healthcare levels in relation to these diseases.

For its complexity, multi-frequency and low systemic involvement, management of systemic autoimmune diseases is a formidable challenge that faces the doctor in clinical practice. His approach requires specific training during the training period, experience and constant updating. Although these diseases are considered rare diseases collectively can be considered unexceptional. "In fact, these diseases affect hundreds of thousands of patients in Spain and primary care physician may face during his tenure multiple cases at least, the onset of the disease and collaborate with specialists monitoring. In this regard, it must therefore learn to recognize and improve their basic skills in managing this process group "

By a group of experts from the two scientific societies: Spanish Society of Family and Community Medicine (SEMFYC) and Spanish Society of Rheumatology (SER). Has designed an index of symptoms and signs guide from each of which has developed an algorithm consensual decision-making leading to a list of possible diagnoses and a proposed referral. Panelists have selected different signs and symptoms suggestive of EAS. Each algorithm has made a couple formed by a specialist rheumatology and other family and community medicine belonging to the SER and SEMFYC, respectively.

22 algorithms were designed with symptoms and syndromes homogenates ordered by organ systems:

- Skin: Rash, photosensitivity, purpura, erythema nodosum and livedo reticularis.

- Mucous: oral and genital aphthae, chronic sinusitis, xerostomia and parotidomegaly.

- Vascular: Raynaud's phenomenon, recurrent venous thrombosis and arterial ischemia.

- Ocular: uveitis, dry eye and episcleritis.

- Neurologic: Headache / temporal arteritis.

- Recurrent Abortions.

- Musculoskeletal: acute arthritis, chronic arthritis, musculoskeletal pain and chronic pain.

- General symptoms: weight loss and fever.

- Laboratory abnormalities: Reactants phase (ESR and CRP), rheumatoid factor, ANAs and creatine.

We present the bypass guide has been developed in an application (App) that facilitates its availability in the current format. The content will be updated conducted according to the evidence presented. It was also released in PDF and the ability to perform it in print will be assessed.
