Stroke Support

People who have had a stroke, their family members and professionals in stroke rehabilitation share their experiences and advice to give you hope, confidence and strategies to live the life you want.  

This platform was developed in partnership between the South Eastern Health and Social Care Trust in Northern Ireland and the social enterprise Bridges Self-Management, which is an expert in evidence-based approaches to co-production and self-management support.  

This app combines peer support and professional advice and can be used on its own as a self-help tool or as an adjunct to rehabilitation sessions with a professional.
