VITAL Connect

VITAL Connect is an app that provides the capability to collect a patient’s vital signs. VITAL Connect captures patients’ vital signs from medical devices using industry-standard Bluetooth connectivity. The VITAL Connect app also has the ability to collect vital signs through manual entry. VITAL Connect allows patients and physicians to monitor chronic conditions from anywhere. For people who may have mobility restrictions or live far from their primary care physician, this can greatly reduce the burden of traveling to medical appointments while still maintaining quality care.

The VITAL Connect app seamlessly and securely transmits vital sign measurements to the VITAL Telehealth Platform. VITAL Connect notifies patients of any changes since their last recording and also alerts patients if their vitals are normal or out of range. VITAL Connect also allows your provider to stay up to date with your progress without consistent appointments in person. The VITAL Connect app also supports patients with multiple health issues. Connect also has secure data transfer for the patient’s data to be sent from patient to clinician. For patients with multiple health issues, or for different vital sign measurements, clinicians can create customized notifications for individual alerts on their patient monitoring.
