DiaNetThe DiaNet App facilitates diabetes care that is based on internationally accepted diabetes care guidelines. This includes “Team Care”, and managing diabetes based on clinical targets that are individually set for every patient (“Treat to Target”).

The system connects the key members of the diabetes healthcare team, including the patient (the main member of the team!), the doctor, the diabetes educator and the funder of care. This team, working together via the Dianet system and App, facilitate improved diabetes health for people with diabetes. This is achieved because the Dianet system promotes and follows international guidelines for diabetes management.

The healthcare team and the patient agree on clinical and activity goals. Achievement of these goals, as well as successfully following care plans and improving diabetes knowledge, form the basis of the Dianet Success Scoreboard.

Direct communication is made possible between the healthcare team and the patient, enabling ongoing mentoring, coaching and day to day support.

Patients can choose which Doctor and Diabetes Educator they prefer to share their data with, knowing that their data is always kept confidential and secure. The Dianet App creates a truly collaborative environment for you and the rest of your diabetes healthcare team to work together for your improved diabetes health.
