The i4We Agent App is a digital tool designed to support Agents, the frontline champions of the Invest4Wellness (i4We) model. Agents act as a bridge between families and the healthcare system, ensuring timely access, accurate follow-up, and trusted support at the community level.

With the i4We Agent App, health facilitators can easily onboard families, guide them through the registration process, and connect them to available healthcare services. The app enables Agents to capture and maintain family and individual health profiles, update vital signs and screening results, and ensure that all data flows seamlessly into the i4We digital health ecosystem.

Key features for Agents include:

Onboarding families: Register up to four members per household with ease.

Digital health records: Maintain accurate health information, prescriptions, and reports.

Screening and assessments: Record vitals like blood pressure, sugar, BMI, oxygen saturation, and temperature.

Appointments and follow-ups: Help families schedule and manage doctor, nurse, and lab visits.

Navigation support: Guide members to services, labs, and community resources.

Reminders and education: Provide regular follow-up, health education, and behavioral nudges to improve adherence.

The Agent App also empowers health workers by providing real-time dashboards and insights, helping them identify families in need of urgent care, track progress of screenings, and ensure no one is left behind.
